Any person, association, partnership or corporation required to obtain a permit under the provisions of this chapter shall make an application to the county department of health services. An annual nonrefundable fee, the exact amount of which shall be determined by the county Board of Supervisors and kept on record by the county department of health services, shall accompany the submission of each application to the department of health services to defray the cost to the county for administering and providing services pursuant to this chapter.
